Hospital in the Home Nursing Teams
Each of the participating hospitals is assigned a dedicated nursing team. These teams consist of five suitably qualified nurses, a key member of this team is the Nurse coordinator, and this individual is the hub for all admissions into the HITH service from that hospital. The Nurse coordinator is “embedded” in the hospital to which his/her team is assigned, this allows the cultivation of key relationships within that specific hospital and in turn facilitates smooth and concise referrals into the programme. A caseload of up to three patients will be assigned to each nurse on the team.
In addition to the Nursing teams the following personnel are also provided in a supporting role:
- Community Physiotherapist.
- Phlebotomists.
- Driver / porter.
- Administrative staff.
This team is contracted on a full time basis to the HITH service.
The Tara Healthcare Senior Nurse Manager Mrs Deirdre Condon will be responsible for arranging meetings, assisting in the implementation of the project on a local hospital level and providing information regarding the project.
